In this example, you can clearly see that this is a vein graft with sluggish flow.  Well after the contrast is injected, the dye 'hangs' in the graft.  Vein grafts are notorious for this because of the 'junk' that accumulates in them, and over time, break off and travel downstream to block the microvasculature.

In this example, while the patient is suffering from an active MI, there is a thrombotic event occurring in the coronary artery.  After the contrast injection ceases, there is no flow in the vessels.  This is likely due to pieces of the thrombosis moving downstream and plugging the microvasculature.
