Key Changes in Case 7 Bradycardia

Pacing is used when the patient fails to respond to atropine or has a high degree block

Dosages for epinephrine 2-10ug/min and dopamine 2-10ug/kg/min may be administered while awaiting a pacemaker

Bradycardia Algorithm

Heart rate less than 60 and inadequate for clinical condition

Maintain patent airway; assist breathing as needed
Give oxygen
Monitor ECG (Identify rhythm), BP, oximetry, and establish IV access
Signs or symptoms of poor perfusion caused by the bradycardia
- Example-altered mental status, ongoing chest pain, hypotension or other signs of shock
If they have adequate perfusion-Observe/Monitor
If they have POOR PERFUSION
- PREPARE FOR TRANSCUTANEOUS PACING: Use without delay for high-degree block (type II second-degree or third degree AV block
- Consider atropine 0.5 mg IV while waiting pacer. If ineffective begin pacing
- Remember you can not give Atropine to someone who has a high degree block
- Consider epinephrine(2 to 10 ug/min) or dopamine (2 to 10 ug/kg/min infusion while awaiting pacer or if pacing ineffective
